CVIH has Two Paths for a Grievance:
1. You can notify CVIH directly of any complaints by completing the CVIH Patient Grievance Form. This form will be forwarded to Administration when you submit through the website. We will work with the departments to resolve any issues within 5 business days. Complaints are summarized monthly for the Quality Assurance Committee.
